At the JD dealers, ask for Allsport - for the other dealers, ask for Turface MVP.

The grit can be had at rural feed stores or grain elevators. Use the online yellow pages and ask for Gran-I-Grit in grower size or #2 cherrystone

I'll leave you to find the bark on your own. It will be easier to find soon as the spring supply starts to arrive. Be sure it is all pine bark or fir bark and the size is appropriate (1/8-1/4").
