I never decide on a first line, either, but the list makes me want to go back to some of my favorites and check out their first lines.

I think Stephen King does think about it. I was googling to find the exact words from The Gunslinger, which I liked:
"The man in black fled across the desert, and the gunslinger followed."
http://www.rousselle.com/allan/archives/000461.html Has a list of his favorite first lines in King novels. After reading some of them I'm thinking yeah, SK does think about this a bit.

Then I though some more and thought, maybe that's just the way he writes. I seem to recall him often writing a zinger of a line to end a chapter.
